Explanation -
Adrenaline is used in combination with xylocaine because
-
It causes vasoconstriction which
1) Maintains the adequate levels of local anaesthetic in the local
area for sufficiently prolonged period (i.e. delays
absorption, prolongs the effect)
2) As the absorption of the xylociane from the site of infiltration
is slow, less dose of xylocaine is required.
This however, is
not applicable at -
A) Penis
B) Fingers & Toes
Because
Dorsal artery of penis is an end artery,
and vasoconstriction induced by adrenaline may lead to
necrosis of glans penis
Similarly, vasoconstriction induced by adrenaline may
lead to ischemic necrosis and gangrene of the finger or
toes.
So, for these
sites, plain xylocaine, without adrenaline is used
