Which one of the following is not likely to be associated with pulmonary fibrosis ?

A) Coal miner's lung
B) Primary sclerosing cholangitis
C) Primary biliary cirrhosis
D) Ankylosing spondylitis

I was confused with B and C and so went wrong...plz your views... Sad

I thought it was Allantoin only to b told by key as Xanthine...cud u pls clarify..

Lung fibrosis

It is important in the exam to be able to differentiate between conditions causing predominately upper or lower zone fibrosis.

Fibrosis predominately affecting the upper zones

• extrinsic allergic alveolitis
• coal worker's pneumoconiosis/progressive massive fibrosis
• silicosis
• sarcoidosis
• ankylosing spondylitis (rare)
• histiocytosis
• tuberculosis


Fibrosis predominately affecting the lower zones

• cryptogenic fibrosing alveolitis
• most connective tissue disorders (except ankylosing spondylitis)
• drug-induced: amiodarone, bleomycin, methotrexate
• asbestosis

A & D are obvious exclusions as they are known causes of upper lobe fibrosis.
Primary biliary cirrhosis is known to cause diffuse intersitial lung disease even to the extend of being granulomatous.PBC is also known to coexist with Sjogrens and Sarcoidosis,both of which are known to cause pulmonary fibrosis.
Therefore the answer is B.
Here is a small tip.
Primary sclerosing cholangitis has an 87% association with ulcerative colitis and UC is treated with sulfasalazine,which is known to cause pulmonary infiltrates and fibrosis.So UC on treatment and not PSC causes fibrosis.

All the options except A)Hypersensitivity are known causes for peripheral eosinophilia.
Check the Table 249-2 in 17th edt Harrison
Pulmonary Infiltrates with Eosinophilia

Allergic bronchopulmonary mycoses
Parasitic infestations
Drug reactions
Eosinophilia-myalgia syndrome

Idiopathic Causes

Loeffler's syndrome
Acute eosinophilic pneumonia
Chronic eosinophilic pneumonia
Allergic granulomatosis of Churg and Strauss
Hypereosinophilic syndrome
