Essential Qualifications for Senior Residents (Hospital Services): M.D. (Medicine/ Paediatrics) for Cardiology, Endocrinology,Gastroenterology, Nephrology, Clinical Hematology and Neurology; M.S. for CVTS, Surgical Gastroenterology included LiverTransplant Unit, Urology, Neurosurgery, MD in respective specialty for Anaesthesiology, Microbiology, Pathology, Radiodiagonosis,Radiotherapy, Nuclear Medicine, Transfusion Medicine. For Nuclear Medicine M.D. (Nuclear Medicine) or MBBS with DRM; ForTransfusion Medicine, M.D. (Transfusion Medicine/ Pathology) or MBBS with Ph.D. in Transfusion Medicine. For Critical CareMedicine MD in Anesthesiology/ Pulmonary Medicine/ Internal Medicine or Pediatric. For Lab Hematology MD in Pathology and ForPediatric Gastroenterology MD in Pediatric or equivalent qualification recognized by Medical Council of India.

Essential Qualifications for PDF: D.M (Nephrology) for Renal Transplantation Medicine (Nephrology), M.Ch. (SurgicalGastroenterology) for Pancreatico-Biliary Surgery (Surgical Gastroenterology). Candidate should submit 200 words write-upto describe work done by him/her in the area, the reason for applying for PDF and what he/she wants to do after PDF. He/sheshould also give names of two referees.

Essential Qualifications for MD Courses: Candidates must have completed one-year rotational internship as on 30.04.2007 afterhaving passed MBBS examination from a Medical College/Institute duly recognized by MCI and should have permanentregistration with MCI.

Age: Upper age limit as on 01.01.2007 is 35 years for PDCC & Senior Residents (HS), and 30 years for MD Courses. Widowed ordivorced or legally separated women and SC/ST/OBC/disabled candidates may be given age relaxation as per Governmentrules. There is no upper age limit for PDF candidates.

General Instructions:
1. Application forms for all programs (except MD) shall be available from 30.09.2006 to 30.10.2006Last date for receipt ofcompleted applications is 31.10.2006.
2. Application forms for MD Courses shall be available from 01.11.2006 to 30.11.2006. Last date for receipt of completedapplication forms is 30.11.2006.
3. The Information brochure cum application form can be downloaded from Institute's Web Site[https://www.sgpgi.ac.in/academic/forms.html] and sent along with a Bank Draft of Rs.1,000/- in favour of DirectorSGPGIMS Lucknow Academic Account, drawn on SBI, SGPGI Branch (Code No.7789), payable at Lucknow.
4. Information brochure cum application forms can also be obtained by hand from State Bank of India, SGPGIMS Branch, Lucknowor by the Office of the Executive Registrar, SGPGIMS, Lucknow, by post on payment of Rs. 1050/- inclusive of postage)by demand draft (non-refundable) in favour of Director SGPGIMS Lucknow Academic Account, drawn on SBI, SGPGIBranch (Code No. 7789), payable at Lucknow.
5. The written examination for PDCC/Senior Residents (Hospital Services) will be held on 03.12.2006 (Sunday). It will be of 90minutes duration & consists of 60 MCQs based on true/false questions. CANDIDATES APPLYING FOR PDF AREEXEMPTED FROM THEORY EXAMINATION.
6. The Theory examination for selection to MD Courses will be held on (Sunday) 28th January 2007.
7. The venue and time of theory examination will be intimated on Admit card
8. The number of seats/posts advertised could vary without prior notice.
9. Reservation will apply as per rules.
10. In case of any dispute, the decision of the Director, SGPGIMS, Lucknow will be final.
